What Forms are needed when applying to HOPE Charter School?


The Criminal History Request Form (SP4-164) is available from HOPE Charter School, Pennsylvania State Police Barracks and from the Pennsylvania State Police web site: Criminal History Request Form . This background check requires a ten dollar ($10) payment in the form of a certified check or money order payable to the Commonwealth of Pennsylvania or to HOPE Charter school if applicant would like to obtain the clearance from HOPE Charter school’s administrative office.

The FBI check requires applicant fingerprints be placed on a special card available from the school in which you are seeking employment. The FBI check requires a payment of twenty-four ($24) in the form of a certified check or money order payable to the Commonwealth of Pennsylvania. All finger print cards must be submitted to HOPE Charter School’s Human Resources office and will be directly sent from the administrative office to the PA. Department of Education for processing.

Child Abuse History Clearance Form (CY 113) may be obtained from HOPE Charter School or the Department of Public Welfare web site at: Child Abuse History Clearance Form  or from the School Services Unit in the Department of Education. The Child Abuse History Clearance requires payment of ten ($10) payable to the Department of Public Welfare in the form of a certified check or money order. Act 151

Academic Certifications/Transcripts


Applicants who are fully certified must provide a copy of their valid certification and must be in good standing regarding requirements of maintaining Act 48 credits.

For teachers needing emergency certification, applicants must obtain official transcripts from their undergraduate and or graduate college/university and submit them with their application in an official sealed envelope from their school. A $40.00 money order made payable to the Commonwealth of Pennsylvania is submitted with the appropriate applications to apply for an emergency certification.


Employees of HOPE Charter School must be free of Contagion disease and provide evidence of such upon hire.
